Please print and complete the following forms and return them to your child's school:

1. Request for Student Records
2. Enrollment Form
3. Acceptable Use of Computer/Internet, e-mail, and cell phone technology
4. Annual Notification to Parents
5. Specialized instructional programs survey
6. Ethnic Survey
7. Home Language Survey (for out-of-district students only)
8. Proof of residency

Please bring copies of the following items to your enrollment appointment:

• Proof of residency documents (see form #8 above)
• Copy of birth certificate
• Immunization record
• Transfer grades (if student is coming from another high school)
• Copy of CAHSEE scores (if not on transcript)
• Unofficial transcript
• District transfer, if out of our district (Middle College students only)
• Caregiver's authorization from our district, if needed (if student lives with someone other than parents)

• California Healthy Kids Survey Notification and Withdrawal Form (grade 7, 9, and 11 only)
• Request to Not Release Information to Colleges/Military Recruiters without Prior Consent

California School Immunization Law

Students entering 7th grade in public and private schools are required to have three Hepatitis B shots , a second MMR shot, and a pertussis booster shot. A Td booster is recommended. The school will ask to see your child's immunization record.
