Please print and complete the following forms and return them to your child's school:
- Acceptable Use of Computer/Internet, E-mail, and Cell Phone Technology
- Annual Verification of Student Residency (Parents who have already submitted documents to their child's school for 2011-12 are NOT required to resubmit)

California School Immunization Law
Students entering 7th grade in public and private schools are required to have three Hepatitis B shots, a second MMR shot, and a pertussis booster shot. A Td booster is recommended. The school will ask to see your child's immunization record.
